Extremely scared of the birth.

Hello Ladies,

I’m currently in my 37th week of my second pregnancy. Aside from some standard pregnancy complaints, it’s been a really nice pregnancy.
However, I haven’t been able to sleep much, if at all, for the past two weeks because I’m full of tension... tension about the birth.

It’s not that I’m afraid of the contractions, the pain they bring, or other crazy things; I managed that last time too, so we’ll just 'do it again'.
My previous birth went smoothly and quickly. At night, I had 'some back pain' that kept getting worse. I arrived at the hospital at 7:30 AM, and the back contractions became increasingly painful. Around 9 AM, my waters broke, followed by a contraction storm that was very intense, and at 13:16, my healthy son was born. So, a smooth birth.

Now, during the pushing phase, I tore terribly—really terribly. Many women say they don’t feel this during birth, but I did feel it.
It was so extreme that even the gynecologist had to consult on how to stitch it up and whether it was even sensible to stitch it.
They didn’t stitch everything because they were afraid of causing more damage and leaving me numb in places where you’d rather not be.

Now, six years later, it has healed, but there’s a missing piece in my inner labia. I still have pain in the area where it was so extremely terrible.

With the next birth approaching, I’m extremely afraid of what might get damaged, how much pain I’ll endure this time, and since they couldn’t say for sure last time whether it would heal well or if I’d still have sensation, I’m completely terrified that this birth will ruin everything.

I’m extremely afraid of tearing again, and I’m dreading the birth a lot.
I also don’t really know what to do about it. Everyone just says, "Oh, it’ll be fine, it worked out last time..." Yes, that’s true, it worked out last time, but you don’t know what I went through.

Does anyone have experience with this or tips?

    Dear girl,

    My tip.. even if that's quite drastic. Request a C-section based on your intense story.
    I sincerely hope that no matter how your labor goes, you'll be spared this!

    Thanks for the replies, ladies. I had a chat with the midwife today and we went over my fears in detail. She completely understood them and tried to reassure me.

    She explained that during a first birth, they sometimes keep the head under tension for a while. By that, she meant that the head is partially out but not completely, and they leave it like that for a bit so the mother can take a breather and the perineum can stretch out slowly.

    However, with some women, the skin is too tight and ends up tearing. Normally, it stretches inward, and in extreme cases, it might find another way, which, unfortunately, is what happened to me.

    I now have to write a birth plan stating my fears and specifying that I don't want the head to be kept under tension, as that's the moment when most women tear.

    I'm still not reassured at all, though, since she admitted that it's quite possible I'll tear again. They don't do C-sections on request, so there's nothing else for it but to try to suppress my fear... although I'm already feeling pain just thinking about it.

    Oh dear, that's really unpleasant.
    I didn't know they did that hopefully they're not doing it again now, because that would be really unpleasant.
    Maybe the birth plan takes away some of your fears. I really hope so! Going into childbirth with anxiety sounds absolutely terrible to me. I myself was looking forward to it so much. It sounds very strange, but at 32 weeks I already had contractions and had to use tocolytics. It was a very difficult pregnancy. I really hope your anxiety decreases!

    Same story here, quite torn up. I work with gynecologists myself and know what the future might bring, like prolapse, etc., but my gynecologist simply said you've already given birth once, so the door is already open. Heard from different doctors that it doesn't necessarily mean it'll happen again with the second one. But it's still exciting.

    It'll probably happen naturally. At that point, I don't think you'll have time to be afraid. And I do think the second time will be less intense. I think an epidural is possible if the fear becomes really extreme, but that's something the OB-GYN needs to assess. I had a C-section myself (due to a small pelvis), and I have to tell you it's not a great solution: with bad luck, it can still hurt (like with me. The anesthesia helped but wasn't comprehensive), and recovery can be slow if you're unlucky (again, like here). So I'm not looking forward to giving birth either, but I know that at that moment it'll just wash over me, so I'll just have to go with the flow. That somehow puts my mind at ease.

    Another option that isn't mentioned here but isn't very nice either is that they give you a small cut. You do get stitched up, and it's not pleasant, but then they'll angle the cut properly so that in the end you won't have any issues from having had a tear at all.

    Maybe you can ask if they'd be willing to do that if they notice the skin is under too much tension, so at least you won't tear.

    From the first contraction to her being born, it took me about 4 hours with contractions that were really manageable, almost too easy. Before I knew it, I was at 10 cm dilation and reached the moment I was so afraid of: pushing. Unfortunately, I hadn’t forgotten how to do it yet, but I didn’t dare to push hard enough. The midwife really had to talk me through it to get me to push. I didn’t dare to put my strength into it and got slightly panicked because I was too scared.
    Finally, I was fed up and got angry at myself, and I pushed through in one go, and she was out!
    And luckily, everything stayed intact this time.
    It’s unbelievable what a difference it made. I still can’t quite wrap my head around it, but this is how it can also go. Besides some tiredness, I feel super and can’t believe I gave birth just a few days ago.

    It's so nice to read your positive message. I hope it'll go well for me too. Throughout the whole pregnancy, I've already been having issues with my lower body because there's more blood flowing through it and it's more swollen. The scars and damage from the first birth are really noticeable. I'm also afraid this birth will make everything worse, but luckily that doesn't have to be the case! I hope so!!

I've already had discomfort in my lower body throughout the whole pregnancy because there's more blood flowing through and it's more swollen. The scars and damage from the first birth are really noticeable. I was also afraid that this birth would make everything worse, but luckily that doesn't have to be the case! I hope so!!

    Try not to let this get in your way during the birth like I did with pushing...
