no clear signs

My little one is 4 weeks old now and doesn't give clear signals regarding hunger, a dirty diaper, needing attention, or being tired. I'm feeding on demand, and it's still 12 feeds a day because he starts shaking his head, sucking on his hand, and smacking his lips every 1-2 hours. But the problem is he does this even when his diaper is dirty or he wants attention. Does anyone have a tip for me, or will it get better with time?

    It comes naturally, at first it's always a bit of a search. My little boy had that too, I kept nursing, but he just had a lot of sucking needs so we eventually gave in to a pacifier and now have a happy little man!!
